In order to rate and quote your account, we need to review the following information:

• Completed Contractors/Consultants Application
• 5 years of currently valued loss runs
• Auto Acord Application with vehicle list
• Garaging location – A MUST
• Driver list with complete license information
• Expiring premium

We also need to review the insured’s safety requirements.

• Does the insured have a written safety program and process for screening and hiring drivers?
• Does the insured pull MVRs? Do they require drug tests?
• Does the insured have a maintenance program in place? A cell phone policy? Any issues with DOT compliance?
